Friday, August 31st, 4:00PM to 9:00PM
Saturday, September 1st, 6:30AM to 10:30AM
All teams MUST check-in at least one hour prior to their first game

What is needed by ALL participating teams at Check-In:

1. Player passes for each player: Current/valid USSF player pass (US Club soccer or USYSA)
2. US Club or IYSA Medical Release Form - you must use the Illinois medical release form.  A copy of each form must be turned in at registration
3. Copy of IYSA Sanctioned Tournament Roster Form
4. Copy of State Association or US Club Approved Team Roster
5. Notification to travel form - From your state soccer association - For out of state teams only
6. IYSA Guest player permit form - If using players outside of your club
7. NT Form - Non-IYSA/US Club Youth Soccer & Foreign Team Agreement - For all US Club teams 

8. NEW REQUIREMENT: Concussion Awareness Training:
   As part of our RISK MANAGEMENT program we require all carded coaches participating in the Schwaben Cup MUST provide proof of concussion awareness training.  If you have not completed the course, use the link below to complete the Heads Up Concussion Training course.  Once you have completed the Heads up Concussion course you will receive a certificate of completion.  Please present a copy of this certificate FOR ALL CARDED COACHES with your registration paperwork.  Coaches without this certificate will not be allowed to participate in the event.
